Community Access provides direct ongoing individualised lifestyle support in accommodation and community access services for adults between the ages of 18 and 65 years. Support arrangments are based around individual needs.

The program provides an outreach service to people living in their own homes and services are designed to:

• assist service users to live independently in their community;
• provide support to access the community;
• facilitate linkages to community based supports, services and options;
• assist service users to maintain their health and wellbeing.
